• Passport-sized photograph of the applicant.
• Proof of identity and address of the applicant (Aadhar Card, Voter ID, Passport, etc.).
• Proof of business premises address (e.g., rent agreement, electricity bill).
• Partnership deed/Certificate of Incorporation/MOA & AOA (for companies or partnerships).
• List of Directors/Partners/Proprietors.
• Food Safety Management System (FSMS) plan or certificate.
• List of food products manufactured or processed.
• No Objection Certificate (NOC) from the municipality or local body, if required.
• Water test report (for businesses involved in food production, processing, or packaging).
• Declaration form.
