- Form B: Duly completed and signed by the proprietor, partner, or authorized signatory.
- Passport-sized photograph of the applicant.
- Proof of identity and address of the proprietor/partner/director.
- Proof of business premises address (e.g., rent agreement, property ownership, utility bills).
- Partnership deed/Certificate of Incorporation/MOA & AOA (for companies or partnerships).
- List of Directors/Partners/Proprietors with contact details.
- Food Safety Management System (FSMS) plan or certificate.
- List of food products manufactured or processed.
- Water test report (if applicable).
- NOC from the municipality or local body.
